ADULT RETRIEVAL HISTORYWord finding and object-location difficulty are not proof of progressive decline.

Adult histories raise questions about retrieval under fatigue, illness, sleep loss, or cognitive load. The record needs repeat testing and the person’s own earlier baseline before a mechanism is assigned.

Why it matters: Measure encoding, retention, retrieval, language access, state, and daily function at the same time.

One adult history describes literacy, good grades, and a social childhood before a major neurologic turning point, followed by a persistent new cognitive and functional baseline.

Why it matters: The community record does not establish progressive decline. It shows why baseline and time matter.
